Using keyword search of BABOK ed. 3, the term business analysis plan crops up only in a few places. None of them directly explain explicitly at which task it is created.
I have to assume it is created in the Business Analysis Planning and Monitoring KA. However, I am not sure if the business analysis plan encompasses all the outputs from all the tasks in the KA, or it is just a part of the Plan Business Analysis Approach task.
Could anyone shed some insights?

I would say your assumption is correct. The BABOK v3.0 states that the purpose of the Plan the Business Analysis Approach Knowledge Area is to define the appropriate method to conduct the business analysis activities and the output is the Business Analysis Approach.
On projects I worked on, the Business Analysis Plan (a project deliverable) is where I documented the Business Analysis Approach with input from the Project Management Plan and other project stakeholders such as the Project Manager.
