EMERGENCY: Need Help Creating/Understanding a Decision Table 

 Hi all,

This is quite an emergency as I have a written test in BA coming up on Friday (5th Oct) and I am struggling to understand a few things.So would REALLY appreciate a speedy response from someone please.

Specifically, I am struggling to understand/create a decision table given specific information/parameters. So I am really hoping someone here can spend a few mins with me, maybe over skype, or just by explaining somethings in simple plain understandable english.

Other things I need help with are Structured text (I need to verify that what I have done is correct) , and also with a CRUD table.
