In the article "Business Analyst Articles: Business Analysis & Systems Analysis", there is a screen print of a matrix, which looks just like what I'm wanting. Does anyone know if a template for this exists and if so, can it be downloaded?

that article? if so that looks like output from a tool (maybe caliber rm?). you should be able to quickly make something like that in excel
